29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch. Welding shop, on or about 26 July 2018 and times prior thereto; the work rest on a Rockwell Pedestal Dual Grinder, Model G10-229-2, Serial number 1225384; was not properly adjusted to a maximum of one-eighth of an inch from the abrasive wheel and work rest. The gap between the abrasive wheel and work rest measured approximately three-eighths of an inch. This condition exposed employees to a caught in hazard. Bureau of Land Management, Alaska Fire Service, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(a)(4) which was contained in OSHA inspection number 978183, citation number 1, item number 6 and was affirmed as a final order on 23 October 2014, with respect to a workplace located at the Bureau of Land Management, Alaska Fire Service at Fort Wainwright, AK.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Building 1535 wash bay, on or about 26 July 2018 and times prior thereto; a Spartan Hot Pressure Washer, Model Fury IIIA, Serial number 61729 had damage to the outer protective jacket on the power cord. This condition exposed employees to an electrical shock and electrical burn hazards. Bureau of Land Management, Alaska Fire Service, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.334(a)(2)(i) which was contained in OSHA inspection number 978183, citation number 1, item number 9 and was affirmed as a final order on 23 October 2014, with respect to a workplace located at the Bureau of Land Management, Alaska Fire Service at Fort Wainwright, AK.

Small engine shop, on or about 26 July 2018 and times prior thereto; an energized two-place electrical outlet had a knockout plug missing on the left-hand side of the outlet box. This condition exposed employees to an electrical shock and electrical burn hazard.
