29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 3174 Oliver Creek Dr. - Employees in the process of decking the 5 in 12 slope (vertical to horizontal) roof, were exposed to a 9'4" fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 8/3/2018.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1324557, citation number 1, item number 1 and was affirmed as a final order on 7/24/2018, with respect to a workplace located at2948 Murray Pass, Odessa, FL.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1291886, citation number 1, item number 1 and was affirmed as a final order on 4/19/2018, with respect to a workplace located at 2653 Whittler Rd., Odessa, FL.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1269907, citation number 1, item number 1 and was affirmed as a final order on 1/25/2018, with respect to a workplace located at 7696 Olive Brook Dr., Wesley Chapel, FL.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1207554, citation number 2, item number 1 and was affirmed as a final order on 5/30/2017, with respect to a workplace located at 3965 Aldea Way, Wesley Chapel, FL.
