29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On Wednesday 12 September 2018 at approximately 12:05 AM, an employee was caught in a machinery pinch point resulting in a leg injury requiring hospitalization. The hospitalization was not reported within 24 hours to the local Area Office of the Occupational Safety and Health Administration. The Austin Area Office was notified on 17 September 2018 at approximately 11:00 AM.
