29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems.  On or about August 21, 2019 and at times prior thereto, maintenance employee(s),were performing safety interlock inspections on top of SMS MEER-RC141 Exit Thread Station #1, that had unprotected sides without being protected from falling, exposing the employee(s) to the hazards of falling from heights that are 4 feet are greater from a lower level.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  On or about August 21, 2019 and at times prior thereto, Axis Pipe and Tube did not require all employees engaged in the servicing of a return flow trench equipped with a built in weir system, that required the lockout or tagout of the Hydrostatic Pipe Tester to control potentially hazardous energy, to affix a personal lockout or tagout device on the equipment, exposing employees to the hazards of unexpected start-up of energy sources.

29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person:  On or about August 21, 2019 and at times prior thereto, TTL employees were required to remove metal scale buildup from inside of a return flow trench equipped with a built in weir system that has the potential to expose employees to an engulfment hazard creating a Permit Required Confined Space (PRCS) without documenting that the permit space had been downgraded to a non-permit through certification.
