29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment. On or about January 29, 2021, and at times prior thereto, authorized employees were not issued and/or assigned a personal lockout or tagout device to be used during the servicing of the Beveller Machine, exposing employees to the hazard of being caught-in moving parts.
