29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about June 6, 2018, at the Converting Pulper Sump Pumps, the employer failed to ensure that employees performing troubleshooting activities were provided with a safe means by which to bleed off/vent hazardous energy in the form of pressure (and heat) which built up in the pump prior to allowing them to open/access it.
