29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: (a) Dock Area at Warehouse 10: On or about August 13, 2020, the stairway employees had to ascend and descend to access the dock had an overgrowth of vegetation. This condition exposed employees to slips, falls, and poison ivy. (b) Dock Area at Warehouse 10: On or about August 14, 2020, the area was littered with items such as but not limited to wooden pallets, chemical totes, bird droppings, and other debris that exposed employees to slip, trip, and fall hazards.

29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition: (a) Dock Area, Warehouse 10: On or about August 13, 2020, the employer did not inspect the loading dock area to ensure the stairs, handrails, and metal plating where employees regularly walk near was safe and free from hazards such as trips and falls from lifted metal dock plates, chipped concrete, and unstable stairs and handrails.
