29 CFR 1910.165(b)(1): The employee alarm system did not provide warning for necessary emergency action as called for in the emergency action plan, or for reaction time for safe escape of employees from the workplace or the immediate work area, or both.  On or about May 13, 2019 and at times prior thereto, the employer did not ensure that the employee alarm system provided time for safe escape of employees from the workplace surrounding and/or the immediate work area where there was a fire.  This exposed the employees to fire and explosion hazards.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of a powered industrial truck (PIT) were performed by the employer without the manufacturer's prior written approval.  On or about May 13, 2019 and at times prior thereto, the employer did not obtain prior written approval from the manufacturer for the modifications and additions made to powered industrial trucks:  a.  The employer modified the PIT by installing a Safecraft fire suppression system to the PIT roll over protection bar behind drivers seat.  This exposed the employees to fire and/or explosion hazards.   b.  The employer modified the PIT by installing titanium exhaust wrap around engine parts of the PIT.  This exposed the employees to fire and/or explosion hazards.

29 CFR 1910.178(q)(6): Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer and by the addition of extra parts not provided by the manufacturer.  On or about May 13, 2019 and at times prior thereto, the employer added a Safecraft fire suppression system to the PIT roll over protection bar behind drivers seat.  This exposed the employees to fire and/or explosion hazards.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, and were placed in service when the examination shows any condition adversely affecting the safety of the vehicle. Defects when found were not immediately reported and corrected.   On or about May 13, 2019 and at times prior thereto, the employer did not ensure that a powered industrial truck (PIT), was removed from service when it was found that it was overheating.  This exposed employees to fire and/or explosion hazards.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition.  On or about May 13, 2019 and at times prior thereto, the employer did not ensure that all areas were provided adequate housekeeping to prevent dust from accumulating onto elevated surfaces. This exposed employees to fire and explosion hazards.  Georgia Pacific was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.22(a)(1) or its equivalent standard, which was contained in OSHA inspection number 1150634, citation number 01, item number 001, and was affirmed as a final order on July 16, 2016, with respect to a workplace located at 604 N.A. Sandifer Highway, Monticello, MS, 39654.
