29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: Company: On, or about, October 20, 2018, the company had not developed and implemented group lockout procedures for task involving multiple employees. When equipment was locked out, only one employee would apply a lock. Procedures such as using a hasp or lockbox were not utilized to allow all exposed employees to observe each lockout point and apply an individual lock to each isolating device.
