29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment. Location: 90 Industrial Park Road, Middletown, CT 06457 - On or about August 18, 2021, the employer did not assess the hazards of the sorting and baling operation and ensure the use of appropriate personal protective equipment during these work tasks. Unacceptable/unsafe materials being sorted and baled can cause struck by and/or skin hazards. Appropriate personal protective equipment will provide head, eye and hand/skin protection.

29 CFR 1910.177(c)(1):The employer did not provide a program to train all employees who service rim wheels in the hazards involved and the safety procedures to follow while servicing rim wheels. Location: 90 Industrial Park Road, Middletown, CT 06457 - On or about August 18, 2021, the employer did not train employees on safety procedures and the hazards involved with handling split rim tires prior to the baling process. As a result, a split rimmed tire was processed in the baler and exploded from the bale upon removal, injuring employees.
