29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Horseshoe Platform: On, or about, January 21, 2026, the Pre Screen/Star Screen was not de-energized and isolated from the power source during the pre-shift inspection that involved opening/removing guards from power transmission equipment. An employee suffered a serious hand injury while attempting to conduct the inspection.
