The oiling and greasing operations are located in direct proximity to a 4 foot by 2 foot hole opening in the Bark and Sawdust Bin.     b. Bark Bin Auger Shaft: On or about October 25, 2018, and at times prior, employees were exposed to fall hazards of approximately 30 feet in the Bark Bin while accessing the platform to perform maintenance activities on the Bark and Sawdust Bin drive motors. The access route had two hole openings which were approximately 5 feet by 3 feet and 5 feet by 2 feet.
