29 CFR 1910.265(c)(23)(i): Guarding. Open bins, bunkers, and hoppers whose upper edges extend less than 3 feet above working level were not equipped with standard handrails and toe boards, or did not have their tops covered by a substantial grill or grating with openings small enough to prevent a man from falling through: a. Bark and Sawdust Bin Auger Motor Drive: On or about October 25, 2018, and at times prior, employees were exposed to fall hazards of approximately 30 feet into a Bark and Sawdust Bin while performing oiling and greasing operations. The oiling and greasing operations are located in direct proximity to a 4 foot by 2 foot hole opening in the Bark and Sawdust Bin. b. Bark Bin Auger Shaft: On or about October 25, 2018, and at times prior, employees were exposed to fall hazards of approximately 30 feet in the Bark Bin while accessing the platform to perform maintenance activities on the Bark and Sawdust Bin drive motors. The access route had two hole openings which were approximately 5 feet by 3 feet and 5 feet by 2 feet.

Bark and Sawdust Bin Motor Drive Assembly: On or about October 21, 2018, and at times prior, an employee suffered an amputation while performing oiling and greasing operation on the energized Sawdust Motor Drive Assembly. Sun Mountain Lumber, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(d)(2), which was contained in OSHA inspection number 1323405, citation number 1, item number 1b and was affirmed as a final order on August 13, 2018 with respect to the workplace located at 181 Greenhouse Road, Deer Lodge, MT 59722.
