29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     On or about November 5, 2018, an employee was not protected from a fall while walking / working from the roof of a commercial building, exposing the employee to a fall of up to 20 feet.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:     On or about November 11, 2018, employees were not protected by personal fall protection equipment while working from an aerial lift, exposing employees to a fall hazard of up to 17 feet.     RMS Properties, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1304675, citation number 1, item number 1 and was affirmed as a final order on September 14, 2018, with respect to a workplace located at 1459 W. Schaumburg Rd., Schaumburg, Illinois, 60194.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:   On or about November 5, 2018, the employer did not ensure employee(s) inspected lanyard(s) prior to working at heights from an aerial lift, exposing an employee to the hazards associated with utilizing a fall arrest system with a defective component.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
