29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a) East end of building, south panel of roof, on or about 11-1-18:  Two employees were installing metal roofing panels on a 4:12 pitch roof and were not protected from falls, exposing the employees to a fall hazard of 14 feet 8 inches.     b) Northwest corner of building, on or about 10-31-18:  Two employees were installing wood trusses and rafters while standing on a sloped roof and were not protected from falls, exposing the employees to a fall hazard of 14 feet 8 inches.  ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:    a) Northwest corner of building, on or about 10-31-18:  Employees were installing wood trusses and rafters while climbing in and on the trusses and rafters, not protected from falls, exposing the employees to a fall hazard of 14 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Southeast corner of building, on or about 11-1-18:  Two employees were accessing the roof via a portable fiberglass extension ladder that was not extended 3 feet above the landing surface, exposing the employees to fall hazards of 14 feet 8 inches.    b) West side of building, on or about 10-31-18:  Employees were accessing the roof via a portable aluminum extension ladder that was not extended 3 feet above the landing surface, exposing the employees to fall hazards of 14 feet 8 inches.
