29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  (a) On or about October 23, 2018, HAC Roofing, LLC, as the exposing employer, did not ensure employees who were required to engage in re-roofing operations on a residential house, were protected from exposure to fall hazards of nine feet and greater.  Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").  HAC Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1264540, citation number 1, item number 1 and was affirmed as a final order on October 5, 2017, with respect to a workplace located at 5025 Lowell Blvd., Denver, CO.  HAC Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1172491, citation number 1, item number 1 and was affirmed as a final order on October 14, 2016, with respect to a workplace located at 1130 Modell Dr, Colorado Springs, CO.  HAC Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1172573, citation number 1, item number 1 and was affirmed as a final order on October 14, 2016, with respect to a workplace located at 5386 Barnstormers Ave, Colorado Springs, CO.
