On 11/1/18, the employer did not conduct a periodic inspection of the Lockout Program on an at least annual basis to correct for deficiencies in the program. The last inspection conducted was January 2017 and procedures were not corrected to indicate where lockout locations were for the energies listed in the procedures or for Affected employees use interlocked doors as a means of protection when performing Authorized work.

On 11/1/18, at the Lavonia facility, Affected employees were performing Authorized work as they used interlocked doors as a means of protection to enter into the Latex line for setup and jam removal procedures.
