OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that the employer did not ensure  that a faulty emergency stop device in the TL6 Latex Line was replaced and was operable, exposing employees to electrical and amputation hazards.  (a) TL6 Latex Line, Perforator; on or about June 21, 2021 and at times prior to, the employer exposed workers to amputation hazards and electrical hazards, in that a faulty emergency stop device in the control panel was removed and was not replaced, preventing the shutdown of the machine during an emergency.

29 CFR  1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) TL6 Machine; on or about June 21, 2021 and at times prior to, employees  clearing  jams and re-starting  the feed inside the cage were exposed to electrical hazards and amputation hazards, where the employer did not ensure that lockout/tagout procedures were utilized to control hazardous energy.

29 CFR  1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:   a) TL6 Machine; on or about June 21, 2021 and at times prior to, the employer exposed employees to  electrical energy, cutting blades, ingoing nip points and rotating parts, in that  locking devices  were not provided  to secure the cage door in the open position for isolation of electrical energy.

29 CFR  1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) TL6 Machine; on or about June 21, 2021 and at times prior to, the employer exposed employees to  amputation hazards, electrical hazards, compressed air,  hydraulic power, and gravitational energy hazards, in that  lockout/tagout training  was not provided to authorized employees performing work inside the machine cage during production operations.   Latexco, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1363634, citation number 2, item number 1, and was affirmed as a final order on 12/23/2019, with respect to a workplace located at 975 Gerrard Road, Lavonia, GA 30553.  Latexco, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1445499, citation number 1, item number 2, and was affirmed as a final order on 01/03/2019, with respect to a workplace located at 975 Gerrard Road, Lavonia, GA 30553.
