29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.                                                                                      On 11/19/19, at the Lavonia facility at the vessels in the facility, employees are exposed to fall hazards as they remove guardrails on the mezzanine level to enter vessels and there is no fall protection around the tops of the vessels exposing employees to fall hazards greater than 18 ft..

29 CFR 1910.147(c)(6)(i)(B): The periodic inspection of the energy control procedure was not conducted to correct any deviations or inadequacies identified.    On 11/19/2019, at the Lavonia job site, the employer did not conduct a periodic inspection of the Lockout program to correct for deviations identified, including on TL10 Latex Line Ovens, the procedure did not include or address the residual heat energy or the length of time for the heat to dissipate or protective equipment needed to guard against the energy.

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.  On 11/19/19, in the Lavonia facility manufacturing area, the employer did not ensure the Lockout/Tagout periodic inspection included a review between the inspector and each authorized employee, of the employee's responsibilities in that not all authorized employees were included in the review and some of the employees did not understand the procedural steps being reviewed and instead did not lockout equipment per the specific written procedures.

On 11/19/19, at the Lavonia facility, the employer did not ensure employees performing service and maintenance to equipment at the facility were trained as authorized persons in the facility's Lockout program prior to conducting those activities.    Latexco, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1363634, citation number 1, item number 2 and was affirmed as a final order on 1/3/2019, with respect to a workplace located at 975 Gerrard Rd., Lavonia, Georgia.
