29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: Latexco, LLC, Lavonia, Ga - In the "kitchen", an employee entered mixing vessel #5 to remove material accumulated at the bottom and on the walls. A permit was not completed prior to entry and all energy sources were not eliminated by locking out the entry of the liquid latex into the tank, on or about September 2019.

Information and training as required by the standard had not been provided, on or about 11/19/19. Latexco, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1) which was contained in OSHA inspection number 133645, citation number 1, item number 8 and was affirmed as a final order on 9/14/16, with respect to a workplace located at 975 Gerrard Rd. Lavonia, GA.
