On or about December 13, 2018, on the residential construction site where an employee was exposed to a fall hazard greater than 6 feet when performing roofing work without a fall protection system

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.    On or about December 13, 2018, at the south side of the residential construction where an employee was exposed to a fall hazard when using an extension ladder to access a roof without extending the ladder side rails at least three feet above the roof landing.
