29 CFR  1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a)	Antonio Martinez. On or about May 28, 2024 at the address of 202 Ordway Ave. Bowling Green OH 43402. The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.  Antonio Martinez was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 1926.20(b)(1), which was contained in OSHA inspection number 1501021, citation number 1, item number 1a and was affirmed as a final order on February 18, 2021 with respect to a workplace located at 653 Pine Valley Dr., Bowling Green, OH 43402.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)	Antonio Martinez. On or about May 28, 2024 at the address of 202 Ordway Ave. Bowling Green OH 43402. The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below.  Antonio Martinez was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 1926.503(a)(1), which was contained in OSHA inspection number 1501021, citation number 1, item number 1b and was affirmed as a final order on February 18, 2021 with respect to a workplace located at 653 Pine Valley Dr., Bowling Green, OH 43402.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)	Antonio Martinez. On or about May 28, 2024 at the address of 202 Ordway Ave. Bowling Green OH 43402. The employer did not ensure employees were protected from falls while performing roofing work activities atop a two-story residential structure. Employees installing Thermoplastic Polyolefin (TPO) roofing membrane were exposed to fall hazards of approximately 19'- 8" to the ground below.  Antonio Martinez was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 1926.501(b)(13), which was contained in OSHA inspection number 1501021, citation number 1, item number 2 and was affirmed as a final order on February 18, 2021 with respect to a workplace located at 653 Pine Valley Dr., Bowling Green, OH 43402.
