On 12/19/2018, Inter Construction LLC operating as the controlling employer for the jobsite at 701-708 Hyde Drive, Ofallon, Missouri.  Inter Construction LLC did not conduct an inspection of the job site, materials, and equipment.  The following hazardous conditions existed:    a)  Employees working for Victor Bonilla did not implement fall protection systems at heights of approximately 21 feet,       exposing the employees to fall hazards.  b)  Employees working for Victor Bonilla were not wearing protective eye wear when operating pneumatic nail guns.  c)  Employees of Nelson Juarez did not implement fall protection systems at heights of approximately 21 feet, exposing         employees to fall hazards.  d)  Employees working for Nelson Juarez were not wearing protective eye wear when operating pneumatic nail guns.
