29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a) On or about March 19, 2019, in the paper cup forming area, maintenance employees were exposed to machine hazards associated with moving parts and pinch points when clearing jams from the area in front of cup forming machine #306 without locking out the machine in accordance with 1910.147(d), or by not implementing alternative measures which provide effective protection in accordance with the exception to 1910.147(a)(2)(ii), such as engaging the machine's E-stop in conjunction with use of specially designed tool to keep the employees' hands away from the zone of danger.
