29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) Dalsin, Inc. at 5000 South Louise Avenue, Sioux Falls, SD 57108: On or about and at times prior to March 27, 2019, the exposing employer did not ensure employees using personal fall protection systems maintained the attachment point of the body harness in the center of the wearer's back near shoulder level. This condition exposed one employee to hazards associated with improper use of fall protection systems. (b) Dalsin, Inc. at 5000 South Louise Avenue, Sioux Falls, SD 57108: On or about and at times prior to March 27, 2019, the exposing employer did not ensure employees using personal fall protection systems maintained the attachment point of the body harness in the center of the wearer's back near shoulder level in that one employee attached the fall protection system to a side d-ring of their body harness. This condition exposed one employee to hazards associated with improper use of fall protection systems.
