29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:  (a)Dalsin, Inc. dba MJ Dalsin Roofing & Sheetmetal at 421 West 8th Street, Sioux Falls, SD 57104: On or about and at times prior to April 16, 2021, the employer did not ensure each employee engaged in re-roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Employees engaged in roofing activities worked near the edge of a commercial flat roof without the use of a fall protection system or a safety monitoring system. This condition exposed employees to a fall hazard of approximately 15 feet.  Abatement Note: The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas:  a)	The nature of fall hazards in the work area; b)	The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; c)	The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used;  d)	The role of each employee in the safety monitoring system when this system is used; e)	The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs; f)	The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection; and g)	The role of employees in fall protection plans; h)	The standards contained in this subpart.  Abatement Note: 1926.503(b)(1) provides that the employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.

Employees were working outside of the warning line system conducting roofing activities without the use of a personal fall arrest system or a safety monitoring system. This condition exposed employees to a fall hazard of approximately 15 feet.  (b)Dalsin, Inc. dba MJ Dalsin Roofing & Sheetmetal at 421 West 8th Street, Sioux Falls, SD 57104: On or about and at times prior to April 19, 2021, the employer did not ensure employees were protected from fall hazards while working on the roof.  Employees were erecting guardrail on the perimeter of the roof while exposed to a fall hazard. This condition exposed employees to a fall hazard of approximately 15 feet. This condition exposed employees to a fall hazard of approximately 15 feet.   Dalsin, Inc. dba MJ Dalsin Roofing & Sheetmetal was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1346961, citation number 1, item number 1 and was affirmed as a final order on October 19, 2018, with respect to a workplace located at 1900 Russell Street, Sioux Falls, SD 57104.
