On or about March 29, 2019, employees performing tear-off and roofing operations from a lower roof were not protected by protective helmets while there was falling debris and objects/materials exposing employees to struck by hazards and head injuries. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
