29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  The employer failed to submit documentation of abatement within 10 days of the abatement date for Citation 1 Item 1.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about August 13, 2025, six employees engaged in roofing activities were exposed to fall hazards greater than 8 feet and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  The employer does not protect each employee on the roof by ensuring that all employees are tied off to their lanyards or by using other fall protection systems. This violation was most recently observed at 230 W Commerce St, Mineral Point, WI 53565. To abate this violation, the employer must ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof without adequate fall protection.   Pursuant to 29 C.F.R. 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from falls.  A&B Remodeling LLC was was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13) which was contained in OSHA inspection number 1810914, citation number 1, item number 1, Issued on 6/5/2025, and was affirmed as a final order on 7/25/2025, with respect to a workplace located at 3145 Muir Field Rd, Madison, WI 53719.  A&B Remodeling LLC was was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13) which was contained in OSHA inspection number 1737987, citation number 2, item number 1, Issued on 9/3/2024, and was affirmed as a final order on 10/1/2024, with respect to a workplace located at 46 Waterford Cir, Madison, WI 53711.  A&B Remodeling LLC was was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13) which was contained in OSHA inspection number 1523444, citation number 2, item number 1, Issued on 7/1/2021, and was affirmed as a final order on 7/22/2021, with respect to a workplace located at 8100 North Beach Drive, Fox Point, WI 53217.  A&B Remodeling LLC was was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13) which was contained in OSHA inspection number 1500287, citation number 1, item number 1, Issued on 1/19/2021, and was affirmed as a final order on 2/17/2021, with respect to a workplace located at 2614 S. Delaware Street, Bay View, WI 53207.  A&B Remodeling LLC was was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13) which was contained in OSHA inspection number 1500287, citation number 1, item number 3, Issued on 5/3/2019, and was affirmed as a final order on 2/17/2021, with respect to a workplace located at 930 high Street, Madison, WI 53715.
