29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:    (a) On or about April 11, 2019, an employee used compressed air nozzle which was not reduced and was approximately 95 p.s.i. at the Haas VF-3B CNC machine (serial no. 1050856) to clean debris off product and the machine.    (b) On or about April 11, 2019, an employee used compressed air nozzle which was not reduced and was approximately 62 p.s.i. at the Haas SL-10 CNC machine (serial no. 3081817) to clean debris off product and the machine.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    (a) On or about April 11, 2019, in the friction welding machine area, metal baskets and a cart were temporarily being stored directly in front of electric equipment, which did not allow access to the electrical disconnect if an emergency occurred within the equipment and electrical power had to be immediately removed.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) On or about April 11, 2019, the Haas VF-3B CNC Vertical Machining Center (serial no. 1050856) had by-passed interlocks which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips.    (b) On or about April 11, 2019, the Haas SL-30 CNC Lathe (serial no. 1050856) had by-passed interlocks which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips.    (c) On or about April 11, 2019, the Haas ST-30 CNC Lathe (serial no. 3091257) had by-passed interlocks which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips.    (d) On or about April 11, 2019, the Caterpillar Inertia Welder WC124 (model no. 250-B, serial no. 44S105) was not provided with adequate guarding to protect employees from injury from rotating parts during operation of the unit.    (e) On or about April 11, 2019, the Caterpillar Inertia Welder WC125 (model no. 250-B, serial no. 44S111) was not provided with adequate guarding to protect employees from injury from rotating parts during operation of the unit.    (f) On or about April 11, 2019, the Caterpillar Inertia Welder WC123 (model no. 250, serial no. 27J91) was not provided with adequate guarding to protect employees from injury from rotating parts during operation of the unit.    (g) On or about April 11, 2019, the Caterpillar Inertia Welder WC145 (model no. 250, serial no. unknown) was not provided with adequate guarding to protect employees from injury from rotating parts during operation of the unit.    (h) On or about April 11, 2019, the Caterpillar Inertia Welder WC116 (model no. 100, serial no. unknown) was not provided with adequate guarding to protect employees from injury from rotating parts during operation of the unit.    (i) On or about April 11, 2019, the Caterpillar Inertia Welder WC121 (model no. 180B , serial no. 59U80) was not provided with adequate guarding to protect employees from injury from rotating parts during operation of the unit.    Spinweld, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1194549, citation number 1, item number 4 and was affirmed as a final order on3/27/17, with respect to a workplace located at W227N546 Westmound Drive, Waukesha, WI 53186.
