Wood shop: on or about 22 April 2019, the employer did not ensure that the Delta Drill Press, serial number R9421, Cat. 70-20 was securely mounted to prevent movement.  This condition exposed employees to a struck by hazard.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage.  Well house: on or about 22 April 2019, two instances were noted where incandescent light bulbs were not guarded:       a)  Inside the well house, and overhead incandescent light fixture measuring six feet four inches from the floor was not guarded.       b)  Outside the well house, adjacent to left side of the entry door; a wall mounted incandescent light fixture measuring six feet six inches from the ground was not guarded.  This condition exposed employees to electrical shock and electrical burn hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  Resources building, boiler room: on or about 22 April 2019, energized exterior wiring was spliced into a plug cord set and attached to a relocatable power tap. This condition exposed employees to electrical shock and electrical burn hazards.

29 CFR 1910.305(g)(1)(iv)(B): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used to run through holes in walls, ceilings, or floors.  Well house; on or about 22 April 2019 and times prior thereto; an energized electrical plug and cord set was used as permanent wiring and routed through a wall opening. This condition exposed employees to electrical shock and electrical burn hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Wood shop: on or about 22 April 2019, an energized 25-foot heavy duty yellow extension cord had damaged to the receptacle plug.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein:  Old Hazmat Storage area: on or about 22 April 2019, an unmarked 55-gallon steel drum barrel containing an unknown chemical substance was not marked identifying the product and the hazard warnings associated with it. This condition exposed employees to chemical burns, poisoning, and skin dermatitis.
