29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:  a)  1310-1326 N. Sable Blvd, Aurora CO 80011:  On or about April 26, 2019 and at times prior, employees were not wearing eye protection while exposed to eye hazards.  Employees were using pneumatic nail guns to install roofing materials and were not wearing safety glasses. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  1310-1326 N. Sable Blvd, Aurora CO 80011:  On or about April 26, 2019, employees were exposed to fall hazards due to portable ladder side rails not extending at least 3 feet above the upper landing surface.  Employees were using a portable ladder to access an upper landing surface 9 feet above the ground.  The side rails extended less than 12 inches above the upper landing surface. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) M&M General Construction, LLC is failing to ensure that its employees are safe from fall hazards during roofing work and other residential construction work including removal and installation of wood roof decking and construction of skylights.  This was most recently observed on April 26, 2019 at 1310-1326 N. Sable Blvd, Aurora CO 80011 when employees of M&M General Construction LLC, were reroofing multi-family residential buildings without fall protection at heights greater than 6 feet.  Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).  M&M General Construction LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1325456, citation number 1, item number 1 and was affirmed as a final order on 08/07/18 with respect to a workplace located at 2443 98th Place, Thornton CO 80229.
