29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) M&M General Construction LLC at 1521 Scott Canyon Ln. Castle Rock, CO 80104: On or before 6/11/21 the employer did not protect employees from injury in that three portable extension ladders did not extend at least three feet beyond the eaves of the roof sections being accessed by employees engaged in residential roofing. This condition exposed employees to fall hazards greater than 10 feet onto concrete.  Abatement Note: Abatement certification and documentation are required for these items. See enclosed "Certification of Corrective Actions Worksheet."

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) M&M General Construction LLC at 1521 Scott Canyon Ln. Castle Rock, CO 80104: On or before 6/11/21 the employer did not protect employees from injury in that employees did not wear fall protection while performing tear off activities on  a 2 story single family home with a roof slope of 5/12. This condition exposed employees to fall hazards of 9, 15, and 18 feet.  Abatement Note: Abatement certification and documentation are required for these items. See enclosed "Certification of Corrective Actions Worksheet."  M&M General Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1422115, citation number 2, item number 1, and was affirmed as a final order on 3/6/20, with respect to a workplace located at 3662 Desert Ridge Place Castle Rock, CO 80108.  M&M General Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1326456, citation number 1, item number 1, and was affirmed as a final order on 8/7/18, with respect to a workplace located at 2443 98th Pl Thornton, CO 80229.
