29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:   a- 19 Congress Street, Lakewood NJ: On or about 05/02/19, the employer did not initiated a program to conduct frequent and regular inspections of the jobsite, to identify hazards and/or stop the work in the event workers were placed in a hazardous condition.  As such, the workers were performing framing and other related work at the worksite without the use of fall protection, without the use of head protection, without providing the required access and egress to the worksite the walking/working surfaces, etc.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  a- 19 Congress Street, Lakewood NJ: Workers, not using fall protection, were exposed to fall hazards in excess of 24 feet to a lower surface when performing floor truss installation, framing and other related activities at the worksite.  Condition observed on or about 05/02/19.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a- 19 Congress Street, Lakewood NJ: On or about 05/02/19 company worksite employees were exposed to struck by hazards when performing framing and other related activities without the use of head protection.     Pumma Construction, Inc. was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1333709, citation number 1, item number 1 and was affirmed as a final order on 10/18/18, with respect to a workplace located at 1 Hastings Avenue, Medford NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 19 Congress Street, Lakewood NJ: Workers, not using fall protection, were exposed to fall hazards in excess of 24 feet to a lower surface when performing floor truss installation, framing and other related activities at the worksite.  Condition observed on or about 05/02/19.   Pumma Construction, Inc. was previously cited for a Repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b) (13), which was contained in OSHA inspection number 1362878, citation number 1, item number 1 and was affirmed as a final order on 03/26/19, with respect to a workplace located at 51 Sullivan Way, Block 15.20, Marlton NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
