Ivex Protective Packaging Inc. located in Sidney, Ohio: On or about May 22, 2019 the employer did not ensure employees deenergized and locked out the Line #1 foam winder prior to starting a new roll of foam an the rotating core, thereby exposing themselves to struck by hazards and rotating equipment.
