On or about May 17, 2019 the employer failed to ensure that the energy isolating devices of the Four-Head (350R2) blow mold rotogranulator were operatied in a manner to isolate the hazardous energies of the equipment prior to performing maintenance. Hazardous energies include mechanical and electrical.
