29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:   a) On or about July 10, 2019 employees engaged in setup activities on the DCM Coater in the Finishing Department, did not utilize energy control procedures prior to the start and were exposed to in-running nip point hazards from the movement of rollers on the varnish deck.   b) On or about May 6, 2019 employees engaged in cleaning tasks on the DCM laminator 83 in the Finishing Department, did not utilize energy control procedures prior to the start and were exposed to in-running nip point hazards from the movement of the coating and impression rollers.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  a) On or about July 25, 2019 the written lockout procedure for the Laminator LM306 did not specifically outline the techniques to render the machine in a zero energy with instructions for relieving and/or dissipating stored and residue energy nor specific steps to verify the zero energy state.   b) On or about July 25, 2019 the written lockout procedure for the DCM Coater did not specifically outline the techniques to render the machine in a zero energy with instructions for relieving and/or dissipating stored and residue energy nor specific steps to verify the zero energy state.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a) On or about July 10, 2019 employees were exposed to machine hazards associated with in-running nip points when they were engaged in set-up activities in close proximity to the varnish deck on the DCM Coater in the finishing department.   b) On or about May 6, 2019 employees were exposed to machine hazards associated with in-running nip points when they were engaged in cleaning activities in close proximity to the coating roll and the impression roll on the DCM laminator 83.   As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)] and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:    a) On or about July 10, 2019 the in-running nip points created between the coating roller and the impression roller were not guarded to prevent employee exposure.    b) On or about September 12, 2019 the in-running nip point created between the pressure roller and the sleeve roller was not guarded to prevent employee exposure.
