29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees were exposed to caught-in hazards. Employees working with and around the following equipment were exposed to injury associated with inadequate machine guarding:  a) Maintenance shop drill press had an unguarded rotating chuck.  b) Maintenance shop band saw had unguarded sections of the unused portion of the blade.  c) A floor fan in the maintenance shop back entranceway had an unguarded rotating shaft.  d) A floor fan in the maintenance shop close to the electrical panels had an approximate 6 inch diameter hole in the center, allowing access to the rotating parts of the fan blade assembly.

The work rest on the bench grinder located in the maintenance shop was not adjusted close enough to the grinding wheel. The distance between the abrasive wheel periphery and the adjustable work rest was approximately three eighths of an inch.

The distance between the abrasive wheel periphery and the adjustable tongue safety guard was approximately half an inch.

One acetylene cylinder was located inside the maintenance shop and a propane cylinder was located outside the maintenance shop dock entrance. Employees working around improperly stored compressed gas cylinders outside the maintenance shop loading dock were exposed to accelerated fire and explosion hazards. Outside the maintenance shop loading deck, oxygen cylinders were stored immediately next to propane and acetylene compressed gas cylinders and were not separated by a 1 hour, 5 foot tall fire rated wall.
