29 CFR 1910.28b)(1): The employer did not ensure that each employee on a walking working surface with an unprotected side edge that was 4 feet (1.2m) or more above a lower level was protected from falling. On or about September 20, 2019 at the Midwest Scrap Management facility- Employee was working and fell from an elevated height while working on a tire shredding machine without any means of fall protection.
