29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest system:    JOB SITE: 1960 Wentzville Parkway, Wentzville, MO 63385, on or about 8/8/2019, workers were observed on top of a 6:12 pitch roof, approximately 23 feet 11 inches above ground level, actively installing black roofing paper and were   not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  The workers were exposed to a fall hazard.    J & H EXTERIORS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1251438, citation number 1, item number 1, issued on 8/14/2017, and was affirmed as a final order on 8/22/2017, with respect to a workplace located at28360 Highway F, Wright City, MO 63390.    J & H EXTERIORS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1187736, citation number 1, item number 1, issued on 2/2/2017, and was affirmed as a final order on 2/14/2017, with respect to a workplace located at 5129 Dominion Drive, Arnold, MO 63010.    J & H EXTERIORS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1065982, citation number 1, item number 1, issued on 6/9/2015, and was affirmed as a final order on 7/16/2015, with respect to a workplace located at 645Castle Ridge Drive, Ballwin, MO 63021.
