Located at 1881 & 1883 Drew Avenue in Columbus, Ohio: On or about August 8, 2019, the employer did not develop and maintain a safety and health program to ensure employees were trained and hazards were identified and corrected onsite while performing roofing work. Such programs included, but were not limited to, fall protection, ladder safety, and personal protective equipment.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. Located at 1881 & 1883 Drew Avenue in Columbus, Ohio: On or about August 8, 2019, employees were removing existing shingles and felt from the first story roof without fall protection, thereby exposing employees to an approximately ten foot fall hazard. Construction By Monroy, LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1316208, citation number 1, item number 1 and was affirmed as a final order on 08/13/2018 with respect to a workplace located at 37830 Chester Road in Avon, Ohio.
