Roofers were using an extension ladder to access the roof of a two story residential home with the ladder extended 18 inches beyond the eave and was not secured to the house. The following condition exposed them to falls up to 12 feet

Roofers were installing shingles and other roofing materials at heights up to 25 feet without fall protection. The following condition exposed them to falls up to 25 feet.  Construction By Monroy, LLC  was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13),  which was contained in OSHA inspection number 1421873, citation number 2, item number 1, and was affirmed as a final order on 09/25/2019, with respect to a workplace located at 1881 & 1883 Drew Avenue Columbus, OH 43235.  Construction By Monroy, LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1316208, citation number 1, item number 1 and was affirmed as a final order on 08/13/2018 with respect to a workplace located at 37830 Chester Road in Avon, Ohio.
