29 CFR 1926.56(a): While work was in progress, construction areas, ramps, runways, corridors, offices, shops or storage areas were not lighted to the minimum illumination intensities listed in table D-3 of this subpart:      (a)   Lallier Construction Inc, at 4550 Kingston Street, Denver, CO: On and preceding 8/13/19, while work was in progress, construction areas, ramps, runways, corridors, offices, shops or storage areas were not lighted to the minimum illumination intensities listed in table D-3 of this subpart.  Lallier Construction Inc directed the work of employees of Mr. Bear Roofing LLC and Industrial Vacuum Service Inc during roofing work of a large commercial building.  Employees of Industrial Vacuum Service Inc removed existing roofing materials on the low slope roof.  Workers performed these operations during nighttime.  Lallier Construction Inc utilized a warning line system that partially enclosed the work area.   Warning lines were not erected from the ladder access point to the area where roof material removal was taking place.  Workers traveled from the active work area to and from the ladder.  The travel path was not illuminated.  This condition exposed employees of Industrial Vacuum Service Inc and Lallier Construction Inc to tripping hazards and an approximate thirty-two foot fall hazard.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8m) from the roof edge:    (a)   Lallier Construction Inc, at 4550 Kingston Street, Denver, CO: On and preceding 8/13/19, when no mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8m) from the roof edge.  Lallier Construction Inc directed the work of employees of Mr. Bear Roofing LLC and Industrial Vacuum Service Inc during roofing work of a large commercial building.  Workers removed existing roofing materials and installed roofing components, including the roof membrane, on the low slope roof.  Workers performed these operations in proximity to the roof edge.  Lallier Construction Inc utilized a warning line system that partially enclosed the work area.  The partial warning line was erected less than six feet from the roof edge.  This condition exposed employees of Mr. Bear Roofing LLC, Industrial Vacuum Service Inc, and Lallier Construction Inc to an approximate thirty-two foot fall hazard.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:    (a)   Lallier Construction Inc, at 4550 Kingston Street, Denver, CO: On and preceding 8/15/19, points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines.  Lallier Construction Inc directed the work of employees of Mr. Bear Roofing LLC and Industrial Vacuum Service Inc during roofing work of a large commercial building.  Workers removed existing roofing materials and installed roofing components, including the roof membrane, on the low slope roof.  Work was taking place in the northeast corner of the roof.  Workers accessed the roof via a fiberglass extension ladder on the south side of the building.  Lallier Construction Inc utilized a warning line system that partially enclosed the work area.  Warning lines were not erected from the point of access several hundred feet to the area where workers were removing or installing roofing.  This condition exposed employees of Mr. Bear Roofing LLC, Industrial Vacuum Service Inc, and Lallier Construction Inc to an approximate thirty-two foot fall hazard.

29 CFR 1910.1200(e)(2)(i): At a multi-employer workplace, the employer's hazard communication program as implemented did not include methods the employer used to provide other employers with on-site access to a safety data sheet for each hazardous chemical the other employers' employees may be exposed to while working:    (a) Lallier Construction Inc, at 4550 Kingston Street, Denver, CO: On and preceding 8/15/19, the employer's hazard communication program as implemented did not include methods the employer used to provide other employers with on-site access to a safety data sheet for each hazardous chemical the other employers' employees may have been exposed to while working.  Lallier Construction Inc directed the work of employees of Mr. Bear Roofing LLC and Industrial Vacuum Service Inc during roofing work of a large commercial building.  Lallier Construction Inc purchased and had brought to the site chemicals used by the subcontractors during the re-roofing of the building.  Lallier Construction Inc did not ensure that its written hazard communication program as implemented included the methods used to provide on-site access to SDSs for each hazardous chemical brought to the site by Lallier Construction Inc that other employers' employees may be exposed to while working at the multi-employer worksite.  Hazardous chemicals brought to the work site by Lallier Construction Inc included, but were not limited to the following:  1.  PVC and KEE HP Membrane Cleaner; and 2.  Bonding adhesives.

29 CFR 1926.852(a): Material was dropped to points lying outside the exterior walls of the structure which had not been effectively protected:  (a)   Lallier Construction Inc, at 4550 Kingston Street, Denver, CO: On and preceding 8/13/19, material was dropped to points lying outside the exterior walls of the structure which had not been effectively protected.  Lallier Construction Inc directed the work of employees of Mr. Bear Roofing LLC and Industrial Vacuum Service Inc during roofing work of a large commercial building.  Workers removed existing roofing materials and installed roofing components, including the roof membrane, on the low slope roof.  Workers dropped waste materials into a dumpster outside the north exterior wall of the building approximately 32 feet below the roof edge.  Neither a chute directed debris into the dumpster nor was a barricade erected around the dumpster.  Large pieces of debris, up to approximately four feet, were dropped from the roof edge.  The debris was distributed on the ground around the dumpster.
