29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: (a)Graff Roofing, Inc. at 305 West 1st Avenue, Lennox, SD 57039: On or about and at times prior to August 15, 2019, the exposing employer did not ensure that the safety monitor was performing fall protection responsibilities related to the roofing activities. The safety monitor was actively engaged in roofing work while three employees were working adjacent to the edge of a commercial re-roofing project. This condition exposed employees to a fall hazard of approximately 15 feet.
