29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: A) Victor Blanco on or about September 5, 2019 at the address of 4587 Clifton Mill Drive, OH: The employer did not ensure the rope's attachment point was secured to the employee's harness at the center of the wears back while installing new ice guard to the residential structure. The employees were exposed to fall hazards of 10 feet to the ground below without their personnel fall arrest system appropriately utilized to provide adequate protection.
