On or about August 17, 2022, at the address of 4446 Bellow Drive, Lorain OH 44053: The employer did not ensure each affected employee used appropriate eye or face protection while utilizing pneumatic nail guns during roofing work activities on residential structure. Employees working in this condition were exposed to potential struck by hazards (flying projectile/ricochet nails).

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system, or personal fall arrest system, nor are employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)	On or about August 17, 2022. The employer ( Victor Blanco) does not ensure that employees were adequately protected from fall hazards while conducting roofing work activities. The violation was most recently observed at 4446 Bellow Drive, Lorain OH 44053, where employees are not protected from a 25 feet fall to the ground below.   To abate this hazard the employer must ensure that his employees performing roofing work are protected from falls, and no employees are allowed onto a roof more than 6 feet high without adequate fall protection.  Victor Blanco was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1599952, citation number 1, item number 2 and was affirmed as a final order on August 12, 2022, with respect to a workplace located at 4839 Westwind Ave Port Clinton, OH 43452.  Victor Blanco was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1460356, citation number 1, item number 1 and was affirmed as a final order on March 20, 2020, with respect to a workplace located at Tradewinds Dr., Catawba, OH 43010.

29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a)	On or about August 17, 2022. The employer (Victor Blanco) does not have a plan or program to identify job site hazards which would assist employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness. The violation was most recently observed at 4446 Bellow Drive, Lorain OH 44053, where employees are not protected from a 25 feet fall to the ground below.   To abate this hazard the employer must initiate, maintain, and communicate an accident prevention program(s) to support employees in identifying and avoiding potential jobsite hazards associated with the employer line of work.   Victor Blanco was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(1), which was contained in OSHA inspection number 1599952, citation number 1, item number 1a and was affirmed as a final order on August 12, 2022, with respect to a workplace located at 4839 Westwind's Ave. Port Clinton, OH 43452. 29 CFR  1926.503(a)(1):The employer does not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)	On or about August 17, 2022, the employer (Victor Blanco) does not ensure that employees were trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below. The violation was most recently observed at 4446 Bellow Drive, Lorain OH 44053, where employees are not protected from a 25 feet fall to the ground below.   To abate this hazard the employer must develop and communicate a fall protection program to support employees in identifying and avoiding potential fall hazards associated with roofing work.
