29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a)	Victor Blanco. On or about June 1, 2022, at the address of 4839 Westwind's Ave. Port Clinton, OH 43452: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.  Victor Blanco was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(1), which was contained in OSHA inspection number 1460356, citation number 2, item number 1a and was affirmed as a final order on March 20, 2020, with respect to a workplace located at Tradewinds Dr., Catawba, OH 43010.  Victor Blanco was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(1), which was contained in OSHA inspection number 1428932, citation number 1, item number 1a and was affirmed as a final order on November 6, 2019, with respect to a workplace located at 4587 Clifton Mill SUBLOT MG-0083, North Ridgeville, OH 44035.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)	Victor Blanco. On or about June 1, 2022, at the address of 4839 Westwind's Ave. Port Clinton, OH 43452: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below.  Victor Blanco was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(a)(1), which was contained in OSHA inspection number 1460356, citation number 2, item number 1b and was affirmed as a final order on March 20, 2020, with respect to a workplace located at Tradewinds Dr., Catawba, OH 43010.   Victor Blanco was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(a)(1), which was contained in OSHA inspection number 1428932, citation number 1, item number 1b and was affirmed as a final order on November 6, 2019, with respect to a workplace located at 4587 Clifton Mill SUBLOT MG-0083, North Ridgeville, OH 44035.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)	Victor Blanco. On or about June 1, 2022, at the address of 4839 Westwind Ave. Port Clinton, OH 43452: The employer did not ensure employees were protected from falls while performing roofing activities atop a single-story residential structure with a 10-12 roof pitch. Employees operating in this conditions were exposed to falls of 22ft. to the ground below.   Victor Blanco was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1460356, citation number 1, item number 1 and was affirmed as a final order on March 20, 2020, with respect to a workplace located at Tradewinds Dr., Catawba, OH 43010.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)	Victor Blanco. On or about June 1, 2022, at the address of 4839 Westwind's Ave. Port Clinton, OH 43452: The employer did not ensure employees using aluminum ladder had their side rails extended at least three feet above the landing surface. Two employees were exposed to fall hazards while using ladder to access a two-story residential structure with a roof eave height of  11 feet from ground below.   Victor Blanco was previously cited for a violation of this Occupational Safety and Health Standard 1926.1053(b)(1), which was contained in OSHA inspection number 1460356, citation number 1, item number 2 and was affirmed as a final order on March 20, 2020, with respect to a workplace located at Tradewinds Dr., Catawba, OH 43010.
