On or about September 19, 2019, at 1526 Shagbark Lane, Bolingbrook, IL, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about September 19, 2019, at 1-5 Black Oak Court, Bolingbrook, IL 60490, employees performing residential roofing operations on roofs, twenty (20) feet and eleven (11) feet, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b.  On or about September 19, 2019, at 1521-1537 Envee Drive, Bolingbrook, IL 60490, employees performing residential roofing operations on roofs,  twenty-one (21) feet two (2) inches and eleven (11) feet two (2) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  c.  On or about September 19, 2019, at 1501-1517 Envee Drive, Bolingbrook, IL 60490, employees performing residential roofing operations on roofs,  twenty (20) feet six (6) inches and eleven (11) feet two (2) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  d.  On or about September 19, 2019, at 1506-1514 Envee Drive, Bolingbrook, IL 60490, employees performing residential roofing operations on roofs,  twenty (20) feet two (2) inches and ten (10) feet four (4) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  e. On or about September 19, 2019, at 1478-1490 Envee Drive, Bolingbrook, IL 60490, employees performing residential roofing operations on roofs,  twenty (20) feet seven (7) inches and eleven (11) feet three (3) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  i.  On or about September 19, 2019, at 1475-1479 Envee Drive, Bolingbrook, IL 60490, employees performing residential roofing operations on roofs,  twenty (20) feet one (1) inch and eleven (11) feet, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  j.  On or about September 19, 2019, at 1518-1538 Shagbark Drive, Bolingbrook, IL 60490, employees performing residential roofing operations on roofs,  nineteen (19) feet six (6) inches and ten (10) feet seven (7) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Raptor Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13) which was contained in OSHA inspection number 1418739, citation number 1, item number 1 and was affirmed as a final order on August 30, 2019, with respect to a workplace located at 13 Astor Circle, Schaumburg, IL 60159.

On or about September 19, 2019, at the above addressed jobsite(s), employees had been provided fall protection training, however, the employer failed to keep a written certification of record.  Raptor Exteriors, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(b)(1) , which was contained in OSHA inspection number 1418739, citation number 2, item number 1 and was affirmed as a final order on August 30, 2019, with respect to a workplace located at 13 Astor Circle, Schaumburg, IL 60159.
