29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a.  On or about April 11, 2025, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about April 11, 2025, at the above addressed jobsite, employees working on the ground, below roofing activities, were not wearing hardhat to protect against falling materials and debris.  Raptor Exteriors, Inc., was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1555486, citation number 1, item number 1 and was affirmed as a final order on February 23, 2022, with respect to a workplace located at 1336 Saint Claire Pl., Schaumburg, Illinois 60173.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about April 11, 2025, at the above addressed jobsite, employees engaged in roofing activities were not protected from fall hazards while working at an elevation of approximately sixteen (16) feet, above the ground, thereby exposing employees to fall hazards.  b.  On or about April 11, 2025, at the above addressed jobsite, employees engaged in roofing activities were not protected from fall hazards while working at an elevation of approximately fifteen (15) feet three (3) inches above the ground, thereby exposing the employees to fall hazards.  c.  On or about April 11, 2025, at the above addressed jobsite, an  employee engaged in roofing activities was not protected from fall hazards while working at an elevation of approximately twenty-one (21) feet  above the ground, thereby exposing the employee to fall hazards.  d.  On or about April 11, 2025, at the above addressed jobsite, an  employee engaged in roofing activities was not protected from fall hazards while working at an elevation of approximately eleven (11) feet four (4) inches  above the ground, thereby exposing the employee to fall hazards.  Raptor Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 1555486, citation number 2, item number 1 and was affirmed as a final order on February 23, 2022, with respect to a workplace located at 1336 Saint Claire Pl, Schaumburg, IL 60173.

On or about April 11, 2025, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall hazards, thereby exposing the employees to the hazards associated with falls.  Raptor Exteriors, Inc.,  was previously cited for a violation of this occupational safety and health standard or its  29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1555486, citation number 1, item number 2 and was affirmed as a final order on February 23, 2022, with respect to a workplace located at 1336 Saint Claire Pl., Schaumburg, Illinois 60173.
