29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  On or about 09/13/2019  Location: On the roof of a two stories new residence under construction at 8883 NW 160 Terr, Miami Lakes, FL,  1) The employer failed to ensure workers used fall protection harnesses while installing tiles, thus exposing employees to a fall hazard of approximately 20 feet to ground level.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  On or about 09/13/2019  Location: On the roof of a two stories new residence under construction at 8883 NW 160 Terr, Miami Lakes, FL,  1) The employer failed to perform inspections of the worksite to determine roof workers were provided with fall protection harnesses, lanyards, anchor points prior to installing tiles, thus exposing the workers to a fall hazard of approximately 20 feet to ground level.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 09/13/2019  Location: On the roof of a two story new residence under construction at 8883 NW 160 Terr, Miami Lakes, FL,  1) Employees were exposed to fall hazards of 20 feet to ground level while performing roofing work and not using any means of fall protection system.    Victoria Roofing Incorporated was previously cited for a serious violation of this standard occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b), which was contained in OSHA inspection number 1255735, citation number 01, item number 001, and was affirmed as a final order on 12/19/2017, with respect to a workplace located at 2234 117 SW 240 Terrace Homestead, FL 33033.
