29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: A) Midwest Elastomers, Inc., located at 700 Industrial Drive, Wapakoneta, OH 45895: On or about September 18, 2019, the employer did not ensure that employees were adequately trained in electrical safety-related work practices such as, but not limited to, the use of special precautionary techniques and personal protective equipment. Employees were exposed to electrical hazards of 277 volts while conducting activities such as, but not limited to, verifying voltage and replacing fluorescent light units with LED light units in the Human Resource Office.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: A) Midwest Elastomers, Inc., located at 700 Industrial Drive, Wapakoneta, OH 45895: On or about September 18, 2019, the employer did not ensure that electrical safety-related work practices were utilized to protect employees from electrical hazards that could result in electrocution, electrical shock, and arc flash/blast. Employees did not de-energize the light units prior to conducted activities such as, but not limited to, verifying voltage and replacing fluorescent light units with LED light units in the Human Resource Office.

Employees were exposed to electrical hazards of 277 volts while conducting activities such as, but not limited to, verifying voltage and replacing fluorescent light units with LED light units in the Human Resource Office.
