Standards in this part requiring training on hazards and related matters, such as standards requiring that employees receive training or that the employer train employees, provide training to employees, or institute or implement a training program, impose a separate compliance duty with respect to each employee covered by the requirement. On October 18, 2019, the employer failed to fully train employees with regard to assuring that combustible dust hazards were fully minimized with deflagration protection, explosion protection, and dust collection equipment.

Midwest Elastomers, Inc. located in Wapakoneta, Ohio: On or about October 18, 2019, the employer did not ensure horizontal surfaces in Building #1 were kept free of combustible dust deposits. The combustible dust deposits are considered explosive based on testing data from the company and bulk samples analyzed by OSHA.

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. a. Midwest Elastomers, Inc. located in Wapakoneta, Ohio: On or about October 18, 2019, the employer did not ensure electrical equipment located outside of the main office entrance near the Y Process line, were approved for a hazardous Class II location. The equipment consisted of ventilated transformers and panelboards which were not dust tight and not rated NEMA type 9 enclosures. b. Midwest Elastomers, Inc. located in Wapakoneta, Ohio: On or about October 18, 2019, the employer did not ensure the Y process control panel located adjacent to the Y process dust collector, was approved for a hazardous Class II location. The control panel was not dust tight and not rated as a NEMA type 9 enclosure. c. Midwest Elastomers, Inc. located in Wapakoneta, Ohio: On or about October 18, 2019, the employer did not ensure the control cabinet for the vacuum system located adjacent to the 9801 process, was approved for a hazardous Class II location. The enclosure was not dust tight and not rated as a NEMA type 9 enclosure
